To prepare your file so that we can present it to the Provincial Government to obtain a CAQ (Certificat d’acceptation du Québec), you must submit the following documents:

  1. Completed Declaration of parental authority form (signed in blue ink).
  2. Colour photocopies of valid passports of both parents and the student.
  3. Photocopy of the proof of insurance with duration of contract (must be on company letterhead). However, to facilitate the process, we now take insurance for your student with a Canadian company. Click here for insurance document.
  4. Photocopy of official birth certificate translated in French or English.
  5. Completed Terms of understanding form with original signature (must not be a photocopy).
  6. Completed Authorization form so that we can retrieve your CAQ.
  7. 4 photos (35 mm x 45 mm) with name of student on recto.
  8. Completed Declaration and Commitments (signed in blue ink)
  9. Completed S.L.I. Application file.

Attention: Files will be submitted only when complete.

Students residing in Austria, France, Hong Kong, Mexico and Monaco must also submit a financial proof:

  1. Bank statements of the last three months.
  2. Official letter from the bank proving your recent financial capacities.


In case of divorce: Provide the proof of legal custody of the student with a copy of the divorce papers.

In case of one deceased parent: Provide a copy of the deceased certificate.
